1. If serving as a distribution center volunteer, please list the days and hours during the month that you are able to serve!
2. If serving as a volunteer shepherd, you are willing to deliver clothing to the clients of One Man's Treasure in the following areas (please list them below). As a volunteer shepherd, you understand that you may decline to accept any request to deliver clothing when requested.
3. All volunteers: I agree to maintain the confidentiality of all clients regarding name, physical location, phone number, and any other information obtained through One Man's Treasure or the client and will not disclose any information about the client to anyone outside of the organization. I will not disclose the location of One Man's Treasure distribution center without the express permission of the organization.
